IT - Lost in Derry 4 Mod / Addon

Mods 1.26 / Mods 1.21

IT - Lost in Derry 4 Mod is a horror-focused add-on that brings in new characters, new structures, and a bunch of scripted moments tied to the IT universe. It’s not just “new mobs,” either—this thing leans hard on events: night encounters, triggered cutscenes, and enemies that don’t always play fair when you get close.

A lot of the chaos comes from who you meet and where you find them. Gray Pennywise shows up through the Hospital structure, and the Hospital can be found in the plain, so even a normal-looking travel day can turn into a problem fast. Watcher Pennywise can appear silently at night just staring from far away, and if you try to close the distance, he vanishes. There’s also a Pennywise Villager—a twisted villager variant that can lurk around villages after dark, moves fast, and explodes on death, so chasing it in a crowded village is basically asking to lose control of the situation. On top of that, several characters (like Georgie, Rich, Mattew, lilly, and more) are tied to cutscenes when you “give” them specific things like fear or a little ship, and those moments can cause new horrors to appear, including things like IT in the blood, Demon baby, and other spawns the add-on calls out.

If you’re the type who wants a full-on boss run, IT - Lost in Derry 4 Mod goes big with Pennywise. Pennywise 2017 is a full encounter where summoning him triggers nightfall, then you fight through multiple forms with custom animations, unique screamers, new attack patterns, and difficulty that ramps as it evolves—ending with a final spider form and Death Pennywise. There’s also a Pennywise 1989 boss that lives in the sewer, with its own set of forms like wolf, cowardly (runs away and spawns balls), wounded, grandmother, and Death Pennywise. Both bosses can drop rare horror artifacts—Pennywise’s Hand or Pennywise’s 1989 Hand—and if you place one in your first hotbar slot you get 10 melee damage as a permanent artifact boost, which can seriously change how your fights feel.

Moment-to-moment, this add-on is all about paranoia and payoff. Red balloon encounters can happen as you explore, with random balloons spawning behind you, and you can also craft your own balloon and place it to make it float there like a warning sign. Fortune cookies add another nasty surprise: when you eat one it writes a prediction and can spawn monsters like fortune_wing, infant_fly, nerve_eye, or fleshy_hatchling, so popping snacks mid-exploration isn’t always “free.” The Painted Woman is tied to a crooked painting event that turns into a two-screamer mini-boss, and Pennywise’s Music Box can be found or crafted—wind it up, watch the cutscene, and brace for the explosive surprise inside before it jumps out and starts dancing. My practical advice: don’t treat this like a casual daylight addon—plan your travel, be ready for night triggers, and if you score an artifact hand, keep that first hotbar slot locked in so you don’t accidentally lose the 10-damage boost during a hectic recovery run.
